Effective March 1, 2005, the Member’s Choice Disability and Life Insurance on loans will be changing. Your disability coverage will be enhanced to a 30-day retroactive program at a rate of $.23 per $100 and an age maximum of 66 years. Your Life coverage will change to $.07 per $100 single coverage and $.105 per $100 joint coverage with an
age maximum of 71 years. If you
choose not to continue coverage,

Information we receive from members on applications or other forms;
Information about members’ transactions with us or others; and
Information we receive from a consumer-reporting agency.
We do not disclose any nonpublic information about members to anyone,
except as permitted by law.
If a member decides to terminate their membership or become an inactive
member, we will adhere to the privacy and practices as described in this
notice.
